MODERN MANAGEMENT OF MENOPAUSE WITH BIO-IDENTICAL HORMONE REPLACEMENT THERAPY
For decades your body made certain chemical structures to ensure the balance of your health. These were the hormones, which naturally decrease with age and the onset of menopause in a woman and andropause in a man. We now understand more than we ever have about the actions of these hormones. However, we would be naive to believe we know the whole story.
Are hormones safe? The honest answer is that no one really knows. We do know there are many benefits to anti-aging and maintenance of our body as we age. Should you take hormones? Only you can answer that question. Learn all you can and then make a decision. However, it makes sense to use the exact hormones your body made. Hence we encourage the use of hormones biologically identical to what your body made.
We encourage you to make an appointment to talk about Bio-Indentical Hormones: or you can begin by reading the chapters in Dr. Komadina’s book.

What is Pellet Therapy?
Pellet Therapy uses hormones derived from natural plant sources to replicate the body’s normal hormonal levels. Patients have found that bio-identical hormone replacement therapy with pellet implants is extremely effective. Implants, placed under the skin, consistently release small, physiologic doses of hormones providing optimal therapy.
Unlike typical oral and transdermal forms of therapy – which produce “roller coaster” hormone levels, resulting in mood and energy fluctuations for the patient -BioTE® Medical Hormone Pellet Therapy, is the only method of hormone therapy that provides sustained hormone levels throughout the day, for up to 4 to 6 months, without any “roller coaster” effect.
Because the hormones used are completely natural, Pellet Therapy is ideal for patients wanting the benefits of a natural hormone, without the drawbacks of a synthetic.
What are Pellets?
Pellets contain a natural plant source of estrogen and testosterone. A compounding pharmacist, using strict federal guidelines, compounds the estrogen and testosterone utilized in the pellets.
These pellets, which are smaller than a grain of rice, are placed in the fatty tissue underneath the skin and most closely mimic the actions of healthy ovaries and testicles with regards to hormone release into the bloodstream. The implantation procedure is easily performed in the office.
Pellet Therapy for Women:
In studies, when compared to conventional hormone replacement therapy, pellets have been shown to be extremely effective for relief of menopausal symptoms, maintenance of bone density, restoration of sleep patterns, and improvement in sex drive, libido, sexual response and performance, relief of depression, anxiety, irritability, mood swings, and many others. Pellet implants have been used to treat migraine and menstrual headaches. It also helps with vaginal dryness, incontinence, urinary urgency and frequency, to name a few. Moreover, hundreds of studies show natural testosterone to be HIGHLY PREVENTATIVE against breast cancer, osteoporosis, Alzheimer’s dementia, to name a few.
Pellet Therapy for Men:
Testosterone levels begin to decline in men beginning in their early 30?s. Most men need to be tested around 45 years of age. Symptoms of testosterone deficiency in men include fatigue, lack of mental acuity, loss of libido, and difficulty achieving, or sustaining erection. It is never too late to benefit from hormone therapy.
Pellet Therapy achieves the sustained levels of testosterone that would be produced by normally functioning testicles. This form of therapy is the only kind that produces the natural level of hormone that men need. Because the testosterone used is totally natural, Pellet Therapy is ideal for men wanting the benefits of a natural hormone, without the drawbacks of a synthetic. Moreover, many studies show optimal testosterone levels in men, over 800, have preventative effects for many disease states such as: cardiovascular disease, Alzheimer’s disease, prostate cancer, osteoporosis, and more.
Typical oral and transdermal forms of therapy produce “roller coaster” blood levels of testosterone, which can result in mood and energy fluctuations for the patient. Rollercoaster levels of hormones do not have the same long term benefits as steady state hormone in the bloodstream.
What if my primary care physician or my gynecologist says that there is ‘no data’ to support the use of pellet implants? And is it safe?
It’s about a patient making an informed choice. Researched and developed since 1939 in the US and Europe, there is a myriad of data to support the ‘long term’ safety and efficacy of hormones delivered by pellet implants. Bio-identical hormone Pellet Therapy is being used safely in 5 continents and has been studied for 8 decades, not only looking at safety and efficacy but the prevention of disease states such as breast cancer, heart disease, osteoporosis, Alzheimer’s dementia, Type 2 diabetes and so much more.
There are hundreds of studies available to practitioners with regards to hormone pellet therapy, unfortunately most do not know where to find them nor take or have the time to research the studies.
Frequently Asked Questions
A: Natural hormones have the same chemical structures as the hormones secreted by the ovaries, adrenal glands, or testes. Therefore they are identical to what your body biologically produces.
A: Some natural hormones come from the yam plant, soybean plant, or Barbasco plant (giant yam).
A: Not usually, most commonly you start in early peri-menopause with a low dose, and gradually increase to an average higher dose in menopause. With time you taper back down the dose to maintain good health while relieving the symptoms of low hormones.
A: Estradiol, testosterone, pregnenolone, estriol, DHEA and sometimes progesterone.
A: Everyone is different. So what is best for one, may not be best for another. Sometimes you need to try 2 or 3 different delivery systems, until you find what works best for you. Troches and sublinguals give about 20% absorption, whereas gels and oil capsules give 10 – 12% absorption.
A: Ideally the best way is to take one half of the daily dose every 12 hours for troche, oral, sublingual and topical doses. However a testosterone troche or sublingual for libido is best to take 2 hours before bedtime. Estrogen for night sweats is best to take with your evening meal. DHEA is best taken after breakfast. Progesterone is best to take at bedtime.
A: Symptoms will usually be relieved equally or more, as long as the patient is balanced properly. Using blood hormone testing will accomplish this as well as monitoring symptoms. We prefer to treat patients…… not lab tests.
A: Yes, when dosed properly the natural hormones will help to prevent Osteoporosis as well as synthetic hormones, if not better.
A: Blood testing for Testosterone, DHEA, Progesterone and Estradiol should be done. If these are low then natural HRT should be used with a testosterone supplement to raise the testosterone levels. This can be in a troche, sublingual, thick gel or topical cream dose form.
A: This is very variable, but the hormones are compounded by a special pharmacy that may not contract with your insurance. However, they are often cheaper than multiple co-pays for multiple hormones all combined into one troche or capsule or cream. The pharmacy will aid you in exploring reimbursement from your carrier.
